On-Demand eLearning: Patient Safety Reporting: Intermediate Course

Targeted to Patient Safety Managers and other Military Treatment Facilities staff already familiar with basic functionalities of the Patient Safety Reporting System, the course is designed to help learners hone the decision-making skills needed to effectively manage patient safety event data in PSR, as part of the ongoing effort to eliminate preventable harm at MTFs.

On-Demand eLearning: Root Cause Analysis

This self-paced module outlines DoD Patient Safety Program’s suggested practices for conducting a root cause analysis, from the initial reporting of a patient safety event through the formation of the RCA Team to the identification of contributing factors and root causes and the recommendation of corrective actions.

Patient Safety Circles
 

In-person or web-based forums focused on a specific topic

 

RCA M&M Series

12 September, 1400 – 1500 ET
Attendees will participate in an interactive forum to discuss key features of conducting an effective and actionable Root Cause Analysis. The speakers will present de-identified RCA cases to generate discussion around strengths and opportunities for improvement in terms of building strong RCAs. Target audiences include all staff involved in a root cause analysis team and Patient Safety Managers.

Patient Safety 24/7 in Emergency Medicine: Fortifying Processes and Mitigating Risks

26 September, 1400-1500 ET
The Emergency Department is a fast paced, high risk area that presents numerous patient safety challenges.  Participants will learn about leading ED patient safety issues that have been reported within the Military Healthcare System and strategies to promote safe care processes through case discussions and ED staff presentations. Target audiences include: CEOs, COOs, CMOs, CNOs, ED Providers, ED Nurses, ED techs, Performance Improvement Directors, Risk Managers, Patient Safety, and Laboratory/Radiology/ staff.

PSR: Tips for Improving Data Quality Series—Focus on Building Reports

10 October, 1400-1500 ET
A recurring series, this forum covers new data quality discussions surrounding patient safety event reporting. This session will focus specifically on generating reports within PSR using examples within the system to demonstrate. Target audiences include: Patient Safety Managers; any user who trends and produces reports on PSR data.
