Medicare Part B and its beneficiaries paid an estimated $69,000 for Xolair that Jurisdiction 14 physicians discarded without documentation in calendar year 2010. The physician practices in our sample that billed for undocumented discard were unaware of the requirement to document discard in the medical record.
